BSA Health & Medical Forms
Parents and Scouts must complete parts A & B to participate in regular camping activities.

Parents and Scouts must complete parts A, B & C to participate in any activity greater than 72 hours in length.  This includes Summer Camp.

Part D must be reviewed by a physician if the scout is participating in a "High Adventure" activity.
